The New Hampshire Transition Community of Practice announces its sixth Transition Summit, The 3 W’s of Planning for the Future, the only statewide conference for training, collaboration, networking, and information, focused on post-secondary outcomes for students. Summit presentations will be focused on effective transition practices that feature one of the following topics areas: job development and employment; Extended Learning Opportunities (ELO); innovative or promising practices in the transition to adulthood and/or effective engagement of students and families in the transition planning process.
